About Our Lady Of Sorrows Catholic School

REGISTRATION: The registration fee is $100.00 per child. This fee is due at the time of registration. Fees are non-refundable unless the child is not accepted at OLS. Acceptance is contingent on the fee payment, submittal of all required forms and information, and the principal's review of all information. Applicants will be notified of acceptance by mail.
SUBSIDY: Many of our local parishes subsidize the tuition of their parishioners who want to send their children to a Catholic school. In doing so, the church agrees to pay a predetermined subsidy amount per student. To be eligible for subsidized tuition rates, you must be an active tithing member of the parish and have an approved subsidy form from your parish on file. This form must be renewed annually. It is the parents' responsibility to send this subsidy application to their pastor for approval. Once the applications are approved, they are forwarded to the school. The school will notify you of approval.

1 child: $3,674.00 per year (11 monthly payments of $334.00)
2 children: $5,676.00 per year (11 monthly payments of $516.00)
3 children: $6,732.00 per year (11 monthly payments of $612.00)
4+ children: $7,579.00 per year (11 monthly payments of $689.00)

2013-2014 NON SUBSIDIZED RATES:
$5,676.00 per child (11 monthly payments of $516.00)

TUITION INFORMATION: Tuition is payable through the Smart Tuition Program: annually, semi-annually, quarterly, or monthly. To set up your account, simply complete and sign the Smart Tuition Payment Agreement and return it to the office.

BOOK RENTAL: Students receive books and workbooks through a book rental program. The fee for this is based on the number of textbooks and workbooks used in each grade level. Book fees for students in grades 1-8 are $150.00 will be billed in 3 installments, September-October-November. Book fees for Kindergarten students are $75.00 and will be billed in one installment - September.

ACTIVITY FEE: Each student will be billed a yearly activity fee of $125.00 which will cover the cost of field trips, weekly publications, art materials, field trip shirts, computer fees, bulk supplies, homeroom activities, class parties, Christmas and end of year teacher gifts, and other items based on the needs of each grade level. Activity fees will be billed in 2 installments, July and August.

FINANCIAL AID: Financial Aid Applications are available for any families interested in applying for financial assistance. Deadline to turn in application each year is March 1. IMMUNIZATION FORMS: Please remember that all students registering for grades K-8 must have immunization forms indicating that they have received the proper number of immunizations required by the State Department of Health. A dose of Tdap (Tetanus, diphtheria, and acellular pertussis) is required for all students entering 6th grade. All forms should be valid for the school year.

SCHOOL SUPPLIES: Supplies will be in the classroom on the first day of school for each student in Kindergarten - 5th grade. Prices vary by grade and will be assessed to your account. Students in grades 6-8 should purchase supplies requested by their teachers. Gr. 6-8 supply list will be sent home in the summer mail-out. All students will need to bring a book bag to school. No rolling book bags for Kindergarten - 2nd grade students.

LUNCH PROGRAM: Our lunch program will be serviced by Christian Catering Service, under the direction of Mrs. Barbara Just. Menus and order forms may be downloaded from the school website www.olsschool.com Make all checks for lunch payable to OLS School. A separate order form should be filled out for each child. Drinks are not included with your lunch order. Peanut butter sandwiches are back on the menu. If your child has a severe peanut allergy, please contact the office. Students may also bring their lunches from home, which may include juice or fruit drinks but not soft drinks or carbonated beverages. No glass bottles should be brought to school. No fast food, please.

DRINK TICKETS: Students in grades 4-8 must purchase a ticket for milk, juice, bottled water and/or ice cream. Students in grades K-3 must purchase a ticket for bottled water, milk and ice cream. These tickets may be purchased in increments of $5.00 ($5, $10, $15, $20, etc.) in the mornings through the homeroom teacher. Please make checks payable to OLS School. All drink items are $.50 each. Ice cream prices are $1.00 each. Please write separate checks for drinks and lunches as they are processed separately.

WEEKLY LITURGIES: The liturgies planned each week by individual classes will be every Thursday at 8:30 a.m. unless indicated otherwise on the school calendar or newsletter (i.e. Holy Days, Thanksgiving, Christmas). Parents are invited and encouraged to attend.

SCHOOL HOURS: Monday: 7:55 a.m. - 2:15 p.m. / Tuesday - Friday: 7:55 a.m. - 3:00 p.m.
Students can arrive as early as 7:30 a.m. Early arrivals in grades K-4 will go to the cafeteria and grades 5-8 will go to the supervising teacher\'s classroom. Students are dismissed to their respective classrooms at 7:45 a.m. with announcements beginning at 7:55 a.m. Students arriving after 8:00 a.m. are late and should sign in at the front office to receive an Admission Pass before going to their classrooms.

EARLY MORNING CARE: Early Morning Care is available from 7:00 a.m. - 7:30 a.m. in the cafeteria. All students dropped off between these hours must report to the cafeteria. The charge is $2.00 for students arriving between 7:00 a.m. - 7:15 a .m. and $1.00 for students arriving between 7:15 a.m. - 7:30 a.m.

MORNING DROP OFF: Morning carpool is ONE LANE only. Please pull up in a single line to the designated area. All children should be ready to get out of the car as it will keep the line moving. For the safety of the children, please do not get out of line and/or pass.

AFTERNOON PICK UP: Please place a nameplate in your window. Parents who wish for their children to enter the right side of the vehicle should be in the right lane of carpool. Those students will be waiting inside the After School Care room until their name is called. Parents who wish for their children to enter the left side of the vehicle should be in the left lane of carpool. Those students will be waiting in their classrooms until their name is called. Please tell your children whether you will be in right or left side of carpool.

AFTER SCHOOL CARE: Care is available at the school each day school is in session for students in grades Wee K4 - 8. Care is available after school until 6 p.m. All students who are not picked up in carpool will report to ASC. No student will be allowed to leave the school grounds unsupervised, unless he/she is walking home. Registration forms and additional information for this program are available in the school office. There is a yearly registration fee of $10.00 per child. Cost is $8.00 per day for 1 child / $15.00 per day for 2 children in a family / $22.00 per day for 3 (or more) children in a family.

ATHLETIC PROGRAM: Athletic programs are available for boys and girls in basketball. Girls' athletics also include volleyball and cheerleading. John Carroll Catholic Middle School Program is available for students in grades 7 & 8 and offer a variety of

UNIFORM POLICY GRADES K5-8: Students in grades K5-8 are required to wear uniforms. Official uniforms are available through Dennis School Uniforms - www.dennisuniform.com / 205-822-7025 or 800-854-6951.
Boys Uniforms - Boys are required to wear white uniform logo shirts with a collar or white turtle neck shirts, navy blue dress pants or navy blue walking shorts and white, navy, or black socks. Socks must be visible above the top of the shoe. Belts are required for students in grades 3-8.
Girls Uniforms - Girls in grades K-5 are required to wear the official uniform jumper or skort. Girls in Grades 6-7-8 are required to wear the official uniform skirt or skort. These uniforms must not be shorter than two inches above the knee. All girls grades K-8 also have the option to wear navy blue pants or navy blue walking shorts. Belts are required with all pants and shorts for students in grades 3-8. White, navy blue or black tights/socks (no leggings) may be worn with jumper, skort, skirt or shorts. Socks must be visible above the top of the shoe. Shorts may be worn under the jumper or skirt but must not be longer than the length of the jumper/skirt. Girls are required to wear uniform logo shirts. Girls K-5 may wear Peter Pan collar shirt (no logo) under the jumper.
Boys and Girls -
Solid white T-shirts may be worn under a uniform shirt. OLS Sweatshirts, fleece jackets and vests with school crest are optional. Jackets/coats worn to school may not be worn in the classroom or during church. All boys' and girls' shirts must be tucked in at all times.

Shoe Policy - School shoes must be low-cut tennis/athletic shoes. No mid or high tops are allowed. Shoes must be white, black, gray or navy (or any combination of these colors) with NO OTHER COLORS. Shoe laces and any accents (labels and trim) should also be white, black, gray or navy. NO OTHER COLORS, no pictures, no flashing lights, etc. Tennis shoes are required for P.E. and wear nicely on our paved playground.

GROOMING: Parents should see that their children are well groomed each day. Extreme hairstyles or coloring for boys or girls are not allowed. Boys' hair must be of moderate length, which is defined as: neatly groomed, out of the eyes in front, not below the bottom of the ear on the sides and not to extend below the top of the collar in the back. Boys are not allowed to wear earrings. No perfume, cologne, or after-shave should be worn to school. For safety reasons, girls may not wear large or
dangling earrings. Any make-up worn by 7th and 8th grade girls should be VERY LIGHT. Make-up is not allowed for students below grade 7.
