OPTION 1 - TOTAL PAYMENT - 5% tuition discount is applied to all accounts paid in full by September 1
OPTION 2 - 2 PAYMENTS - 3% tuition discount is applied when tuition is paid in 2 payments; ½ payment
by September 1 and ½ payment by January 1
OPTION 3 - 9 PAYMENTS - Tuition is split into 9 equal payments, due by the first of each month,
starting September 1 and ending May 1

ENROLLMENT AGREEMENT
1. It is understood that no refunds will be made for school closings, sickness, withdrawals or other absences except as outlined in
the œmake-up day  plan.
2. Before August 1st, except for a $25 per child service fee, deposits are refundable for any reason. After August 1st, there will be a
$50 service charge for any withdrawals. A pro-rata refund will be made with a 30 day written notice of withdrawal.
3. Permission is hereby granted for Merrick Woods Country Day School to provide medical attention for a child in case of
emergency, when a family physician cannot be reached.
4. Permission is hereby granted to take my child on educational field trips via Merrick Woods bus without further consent.
5. Parent consents to use of child's photograph in advertising and promotion.
6. In the event of unforeseen circumstances causing extraordinary expenses, Merrick Woods reserves the right to impose a
surcharge or increase in tuition with 30 days notice.
7. Merrick Woods Country Day School reserves the right to terminate agreement at any time at the discretion of the director.
8. Late charges of 1½% per month will be added to all past due accounts. In addition, you agree to pay all legal fees (equal to the
greater of $300 or 1/3 of the delinquent amount plus interest) and costs incurred in collecting delinquent accounts.
9. It is agreed that any dispute concerning, relating, arising out of or referring to the subject matter of this contract shall be
resolved exclusively by binding arbitration in Nassau County, New York according to the then existing commercial rules of the
American Arbitration Association and the substantive laws of that state.
10. All school transportation services will be provided by Merrick Woods bus unless indicated otherwise.
