NEW STUDENT APPLICATION REQUIREMENTS
1. Complete and return green application form.
2. Attach a copy of applicant's last report card (not required for kindergarten.)
3. Pay $50.00 Application Fee (NON-REFUNDABLE)
4. Present copy of applicant's birth certificate for age verification.
5. An admissions test will be scheduled for the applicant after requirements 1-4 above have been met.
6. Bring your child to the admission test (on scheduled date).
7. Wait for notification that your child has been ACCEPTED FOR ADMISSION.

REGISTRATION CHECKLIST
YOU MUST REGISTER TO RESERVE YOUR CHILD'S PLACE IN THE SCHOOL.
1. Pay $250.00 Registration Fee (REFUNDABLE THROUGH MAY 31, 2012)
2. Complete the registration and financial agreement forms.
3. Provide completed immunization record (Students are required to have up-to-date immunizations before they may attend class).
4. Return CHDP health certificate (First grade students only).
5. Congratulations, your registration is complete!

TUITION AND FEES
Yearly tuition is $5100 per student which is based on an estimated annual expense of $5100 per student. Families with multiple students are offered a 10% optional discount. Money from donations may be used to cover any deficit resulting from the discounts.

TWO CHILDREN
ANNUAL (MONTHLY) TUITION WITH DISCOUNTS: $ 9,690.00 ($969)
ANNUAL (MONTHLY) TUITION WITHOUT DISCOUNT: $10,200.00 ($1020)

THREE CHILDREN
ANNUAL (MONTHLY) TUITION WITH DISCOUNTS: $14,280.00 ($1,428)
ANNUAL (MONTHLY) TUITION WITHOUT DISCOUNT: $15,300.00 ($1530)

FOUR CHILDREN
ANNUAL (MONTHLY) TUITION WITH DISCOUNTS: $18,870.00 ($1,887)
ANNUAL (MONTHLY) TUITION WITHOUT DISCOUNT: $20,400.00 ($2040)
