Enrollment is complete once the office receives the following:

Enrollment Packet filled out in its entirety.
A copy of the student Birth Certificate. This certificate is used to verify the correct legal name and birth date. In compliance with federal, state, and school guidelines, all students are enrolled using the legal name on the student's birth certificate UNLESS there has been a court-ordered name change and a photocopy of the name change has been provided at enrollment. You have 30 days to provide a certified copy of the birth certificate or other reliable proof of identity.
Copy of Immunization records. All students entering any Arizona school are required by law to be immunized prior to the start of classes. Proof of immunizations (printed from your doctor's office) or a signed waiver is required at the time of enrollment and must include the name of the person, the birth date, the type of vaccine administered, and the month, day and year of each immunization.Requirements are dependent upon the grade level and age of each child. Most recent Standardized Test results (if you have any).
Withdrawal form from previous school (during school year enrollment only).
Certificate showing promotion to grade enrolling (during summer enrollment only.
Placement Tests: All placement tests only place students at an appropriate level. They are not used to determine grade level. Placement tests will be given during the first week of school.
Tuition Payment

You can complete enrollment in two ways:
In person: Stop by the Al-Huda School office to fill out a copy of the enrollment application in person.
Hard Copy: Download or directly print the Enrollment Packet (below), fill it in its entirety, and submit to our office.
