GSHS ADMISSIONS PROCEDURES AND REQUIREMENTS
The G. Stanley Hall School keeps a copy of the Policy and Procedure Manual on-site at the
school, which includes this admissions procedure.
The Program Director and/or Educational Director provides a written notice to all parents that
copies of all policies and procedures can be made available upon request of the parent/guardian.
Admission protocol and criteria for the G. Stanley Hall School is as follows:
A. Phone contact with the Program Director at G. Stanley Hall is made by the referring
school district. A general review of the student's record and IEP are conducted during
this contact. General appropriateness of the referred student is often determined via this
first step.
B. The Director of the G. Stanley Hall School informs the school district of the requirement
of written material to move the referral process along. The following is what is requested
including:
1. Educational Plan
2. Academic record
3. Psychological evaluations (if any)
4. Intelligence, learning disability and projective test results (if any)
5. Psychological history
6. Appropriate release of information forms
7. Other appropriate information
C. If record material indicates appropriateness, admissions interview with the child and
parent/guardian is scheduled.
Generally requested to attend:
1. the students
2. parents or guardians
3. referring school personnel
4. involved DCF, DMH personnel
The purpose of the initial interviews is as follows:
1. To describe educational/clinical treatment program.
2. To answer student, parent, guardian, public school person, or human service
agency representative questions.
3. To ask questions to student and parent or guardian to determine appropriateness
of student for the G. Stanley Hall School.
4. To provide a tour of the G. Stanley Hall School.
5. To provide information regarding the residential programs, if applicable.
D. Following the interviews, the G. Stanley Hall School Leadership Team meets to
determine referred students status. If accepted, student and other appropriate individuals
are advised, and an admission date is decided upon. The following required
documentation has to be sent to the School Director before an intake date is agreed upon:
1. Most recent physical examination signed by a licensed Physician (must
be dated within one year).
2. Up to date Immunization Record, and encouragement but not required
TB testing within last year.
3. Current doctor's orders with current medication (including all
medications) list signed by the prescribing doctor and dated within 30 days of the
admission date.
4. 30 day supply of medications has to be brought to the school by a
parent/guardian
E. Prior to or at the time a student enters the G. Stanley Hall School, an orientation is held.
A parent or a guardian is required to attend this meeting before a student may begin
attending school. Representatives from the student's local public school and/or from any
human service agency serving the student may attend the interview, if they wish.
The purpose of the orientation is:
1. To answer questions that parent's or student's have that were not answered in the
initial interview.
2. To provide the parents with an Admissions Handbook including the G. Stanley
Hall School's policies and procedures as required by the Department of
Education.
3. To provide information to the parents regarding the health care program.
4. To obtain the following information from parents regarding their child:
A. Legal status of student
B. Record of student's immunization
C. Medical history, including known allergies of student
5. To get the following permission forms signed by parents:
A. Medical Treatment Authorization
B. Reflection Room/Physical Restraint
C. Administration of Psychotropic and prescription drugs (if applicable)
D. Field Trip Permission Form
E. Psychological and Educational Testing
6. To give students and parents an opportunity to meet staff and other students.
F. In the event of emergency placements, the G. Stanley Hall School shall make provisions
for a complete examination of the student within 30 days of admission.
